Uranium One Exploration, Inc. owns and operates the Velvet Mine, which is an underground uranium and vanadium mine. The intercepted mine water is pumped and mixed with barium chloride and then up to an initial treatment tank where the barium chloride assists in Radium reduction. Project Description: On August 8, 2008, Tesoro Refining and Marketing (Tesoro) received approval from the Environmental Protection Agency (EPA) to use an alternative monitoring plan under 40 CFR Part 60 Subpart J for the Fluid Catalytic Cracking Unit (FCCU). As part of EPA's approval, Tesoro is required to incorporate the alternate plan into their Approval Order (AO). The change is subject to public review and comment. In addition, Tesoro's AO will be revised in accordance with Utah Division of Air Quality's new permitting template. Two conditions will be modified as a result of the AO change. Former AO Condition 11 will list the approval letter of August 8, 2008. Former AO Condition 12 will be modified to change two monitoring values - in paragraph B the lower temperature range bound of the FCCU Regenerator will be reduced from 1256 to 1182 degrees Fahrenheit, and in paragraph D the CO concentration value will be increased from 3.0 to 4.4% by volume. No other changes will be made to the conditions of the AO outside of the new formatting and numbering imposed by the permitting template. No changes in emissions will result from this AO revision.

Project Description: Consolidated Energy has submitted a Notice of Intent to construct a 109 MW co-generation power plant in Davis County, Utah. The proposed facility will consist of one large two-stroke engine operating on residual fuel oil, one 814 MMBtu boiler fired with petroleum coke, and two natural-gas fired auxiliary boilers (rated at 126 and 205 MMBtu). The residual fuel oil will be provided by the Holly Refinery located just to the east of the proposed facility's site, while the petroleum coke will be shipped in from off-site. Residual steam from the boiler process will be sold back to the Holly Refinery, although all electricity will be provided to the general power grid. Consolidated Energy proposes to limit the source's hours of operation in order to permit the source under the Synthetic Minor Source classification. The source will be located in Davis County, which is a maintenance area for ozone. No offsets for this source are required given the source's synthetic minor classification and VOC emissions less than 50 tons/yr.
Total emissions, in tons per year, are limited as follows: PM10 60.9; NOx 98.1; SO2 97.8; CO 98.0; VOC 49. Emissions of HAPs shall be limited such that no single non-metal HAP shall exceed 8.67 tons/yr, no single metal HAP shall exceed 0.92 tons/yr and no combination of HAPs shall exceed 9.59 tons/yr.

The source is located in Utah County, near the city of Lehi, Utah. Utah County is a Non-attainment area of the National Ambient Air Quality Standards (NAAQS) for PM10. There will be no violations of the NAAQS resulting from this project.
IM Flash Technologies, LLC is subject to NSPS Subpart Dc (Standards of Performance for Small Industrial-Commercial-Institutional Steam Generating Units). Because the source is not a major HAP source, it is only subject to the record keeping and reporting requirements in the subpart. According to the EPA memo dated April 19, 1999 entitled “Title V applicability of one time reporting provisions for nonmajor sources”, and the Division of Air Quality Guidelines dated February 2000 entitled “Applicability of Title V to sources subject only to certain record keeping requirements of Federal Standards viz. NSPS and MACTs”, Title V does not apply to this source because these generic record keeping and reporting requirements by themselves are not limits or standards. National Emission Standards for Hazardous Air Pollutants (NESHAP) and Maximum Achievable Control Technology (MACT) regulations do not apply to this source.
The source has limitations that are established in the Intent to Approve. Visible emissions are controlled by opacity standards set in the ITA. Some of the limitations that are listed in the ITA are included in this newspaper notice and are as follows:
All baghouses - 10% opacity
All packed bed scrubbers - 10% opacity
All boilers - 10% opacity

Fairview City completed construction of a new wastewater treatment plant in late July 2005 and started discharging. The facility has a design capacity of 0.3 million gallons per day (MGD). It is a Membrane Bioreactor wastewater treatment system, serving a population of approximately 1650. The permit contains Biosolids and Storm Water components.

New Source Performance Standards (NSPS) 40 CFR 60 Subparts 000 and IIII regulations apply to this source. Maximum Available Control Technology (MACT) 40 CFR 63 Subpart ZZZ regulations apply to this source. National Emission Standards for Hazardous Air Pollutants (NESHAP) regulations do not apply to this source. Title V of the 1990 Clean Air Act applies to this source.
The source has limitations that are established in the Intent to Approve. Visible emissions are controlled by opacity standards set in the ITA. The limitations that are listed in the ITA for the new equipment include:
Opacity shall be limited to the values stated below and water sprays shall be required for the control of fugitive dust.

B. All screens – 10% opacity
C. All conveyor transfer points – 10% opacity

Cargill Salt (Cargill) draws water from the Great Salt Lake, and by the process of evaporation, separates out the salt. Cargill Salt has the ability to discharge bitterns back to the Great Salt Lake, however, all bitterns from Cargill Salt are conveyed to Magnesium Corporation of America via a ditch for further processing.

Worthington Foods, Inc. operates a food processing plant at the Freeport Center located in Clearfield, Davis County. The county is a maintenance area for ozone. The company has requested the following modifications:
(1). To add a Fryer #4;
(2). To add a thermal fluid heater #2 to provide heat for the Fryer #4;
(3). To expand the capacity of Fryer # 3, and
(4). To increase natural gas consumption from 347.38 MMscf to 438.33 MMscf per rolling 12-month period.

Title V of the 1990 Clean Air Act does not apply to this source. The emissions, in tons per year, will change as follows: PM10 (-) 6.44 and NOx (+) 1.10, SO2 (+) 0.01, CO (+) 2.38, and HAPs (+) 0.08. The changes in emissions will result in the following, in tons per year, potential to emit totals: PM10 = 7.28, NOx = 22.62, SO2 = 0.17, CO = 28.37, VOC = 49.73 and HAPs = 0.68.

The Deseret Chemical Depot (DCD) will hold a public information meeting on December 3, 2008, at 6:00 p.m. at the Tooele Chemical Stockpile Outreach Office for a Class 3 Modification (08.03378) to its Tooele Chemical Agent Disposal Facility (TOCDF) hazardous waste management permit. TOCDF proposes to install and operate an Autoclave for treating a portion of the inventory of secondary waste that has been generated throughout the demilitarization of the DCD chemical agent stockpile within an igloo in Area 10. The modification request includes a test plan that will be performed to demonstrate the effectiveness of the Autoclave treatment process and establish regulated operating parameters that will be specified in the TOCDF RCRA Permit.

TOCDF proposes to install a Drum Ventilation System (DVS) which will consist of 1) two enclosures sized to house six drums each (DVS Enclosures), 2) one enclosure sized to enable two personnel entrants in addition to four drums (DVS Sorting Room, DVSSR), and 3) a carbon adsorption filtration system. The DVS will enable the actual agent-contamination status of approximately 15,000 secondary waste containers to be determined via monitoring of the drum’s interior.

Barnes Aerospace, Ogden Division, is a manufacturer and repairer of engineered components for aircraft engines, airframes and land-based industrial gas turbines. The company has requested an expansion of its operations. The new operations will include a pretreat spray booth, tanks, scrubbers, a curing oven and a diesel powered standby generator. The expansion will be at a new location at 1025 Depot Drive in Ogden in Weber County. Ogden is a Non-attainment area of the National Ambient Air Quality Standards (NAAQS) for PM10 and CO. New Source Performance Standards (NSPS), National Emission Standards for Hazardous Air Pollutants (NESHAP) and Maximum Achievable Control Technology (MACT) regulations do not apply to this source. NOTICE OF PUBLIC COMMENT
The Utah Division of Solid and Hazardous Waste has completed a review of a used oil transporter permit application filed on May 8, 2008, by Veolia ES Technical Solutions, LLC (Veolia), with supplemental information received through November 26, 2008. Veolia is headquartered in Milwaukee, Wisconsin, and has an office located at 709 North Taylor Way, Suite 1, North Salt Lake City, Utah.
Under this used oil transporter permit, Veolia would be allowed to transport containers of used oil in Utah in box vans, pick-up trucks, and tractor/trailers. Veolia would not be permitted to store used oil in excess of 24 hours without first obtaining a used oil transfer facility permit from the Executive Secretary of the Utah Solid and Hazardous Waste Control Board.
