Groundwater Discharge Permit No.UGW270001 is being renewed for the existing Facility and Tailings Pond owned by Brush Resources. All plant wastewater and tailings are disposed of in a 220-acre tailings pond at the plant site. This tailings pond has received all of the liquid wastes from the mill facility since 1969. A Discharge Minimization Technology (DMT) has been approved to reduce the seepage rate and to protect groundwater from potential contamination.

Uranium One Exploration, Inc. has submitted a NOI to reopen an existing uranium and vanadium mine located in San Juan County. The company plans to restart the mine involving mining up to 750 tons per day and 255,500 tons per year of uranium and vanadium ore and waste rock. The principle pollutants will be PM10 from material handling activities and stockpile areas, and VOC from three storage tanks.
San Juan County is an attainment area of the National Ambient Air Quality Standards (NAAQS) for all pollutants. National Emission Standards for Hazardous Air Pollutants (NESHAP) 40 CFR parts 61 subpart B (National Emissions Standards for Radon Emissions From Underground Uranium Mines) applies to this source. New Source Performance Standard (NSPS) and Maximum Achievable Control Technology (MACT) regulations do not apply to this source. Title V of the 1990 Clean Air Act applies to this source.
The emissions from the operations will result in the following, in tons per year, potential to emit totals: PM10 = 13.76, NOx =3.15, CO = 1.81, VOC = 0.25 and Hazardous Air Pollutants (HAPs) = 0.05.
The completed engineering evaluation and air quality impact analysis showed that the proposed project meets the requirements of federal air quality regulations and the State air quality rules. Approval is recommended.

Fresenius Medical Care is requesting to expand two dialyzer fiber lines (#9 & #10) and a dialyzer assembly line (#6), an additional 800 kW emergency generator, one single packed column tower wet scrubber, several new storage tanks, and replacement of an existing 200 hp boiler (#1) with a 500 hp boiler (#12). Fresenius Medical Care is the owner and operator of the medical device manufacturing plant located in Ogden, a non-attainment area of the National Ambient Air Quality Standards (NAAQS) for PM10 and is a maintenance area for CO.
New Source Performance Standards (NSPS) - 40 CFR 60 Subpart A, Subpart Dc, Subpart Kb, and Subpart IIII apply to this source. National Emission Standards for Hazardous Air Pollutants (NESHAP) regulations and Maximum Achievable Control Technology (MACT) do not apply to this source. Title V of the 1990 Clean Air Act applies to this source as an area source. This area source does not require a Title V operating permit.
The emissions, in tons per year, will change as follows:
PM10 + 0.59, SO2 + 0.23, NO2 + 3.57, CO + 6.11, VOCs + 6.42, and HAPs + 0.14

The Tooele Chemical Agent Disposal Facility (TOCDF) will hold a public information meeting on July 15, 2009, at 6:00 p.m. at the Tooele Chemical Stockpile Outreach Office for a Class 3 permit modification request (09.01819). The Chemical Agent Munitions Disposal System (CAMDS) has completed its operational mission and is currently in the process of attaining formal RCRA Closure, which includes decontamination, equipment removal, and building removal as specified in the CAMDS Closure Plan. Until the CAMDS facility has been dismantled to the extent required for RCRA Closure status, the CAMDS RCRA Permit is required to remain effective. On 9/24/09, the CAMDS Permit expires. Rather than renewing the CAMDS Permit as a continuing separate entity in the RCRA Permit realm, it is proposed to incorporate into the TOCDF Permit those elements of the CAMDS Permit required to support continuing CAMDS work.

The Executive Secretary of the Utah Solid and Hazardous Waste Control Board is proposing to issue a Hazardous Waste Storage and Subpart X Thermal Treatment Permit to ATK Launch Systems (ATK) and the United States Navy for the operations at the Naval Industrial Reserve Ordnance Plant (NIROP) facility. NIROP is owned by the United States Navy and operated under contract by ATK which owns and/or operates the rest of the Bacchus Facility. NIROP has operated under interim status since 1980. The Permit will authorize the Permittees to use two hazardous waste storage units and a Subpart X thermal treatment unit in accordance with R315 of the Utah Administrative Code. Intermountain Health Care has requested a modification to their Approval Order (AO) to add an outpatient clinic to the Utah Valley Regional Medical Center. The emitting units from the outpatient clinic will be four boilers and one emergency generator. The boilers will use natural gas as a primary fuel and diesel fuel as a backup fuel. The emergency generator will use diesel fuel and will be limited on the annual hours of operation.
The facility is located in Utah County, which is a non-attainment area of the National Ambient Air Quality Standards (NAAQS) for PM10 and is a Maintenance area for Carbon monoxide (CO). New Source Performance Standard (NSPS) Subpart A, Dc, and IIII, and MACT Subpart A and ZZZZ apply to this source. This source is a Title V Area Source.
Emissions previously calculated for the facility included emissions from mobile sources in the parking area. Emissions from mobile sources were not included in this AO and the parking area was removed from the equipment list. Hazardous Air Pollutant (HAP) emissions from the boilers and engines were not included in the previous AO. These HAP emissions will be included in this AO. Due to the additional equipment and removal of mobile source emissions, the potential emissions, in tons per year, will change as follows: PM10 + 0.90, NOx + 5.06, SO2 + 1.18, CO - 20.27, VOC + 1.10, HAP + 0.68.
The changes in emissions will result in the following, in tons per year, potential to emit totals: PM10 = 2.73, NOx = 30.92, SO2 = 7.59, CO = 16.29, VOC = 11.08, HAPs = 0.78.

This Approval Order (AO) Modification is for Ames Construction, Inc.'s portable equipment AO DAQE-AN0119410009-08. This AO modification is for the addition and deletion of portable aggregate processing equipment found in this AO. Sets of equipment chosen from the equipment approved in this AO shall be temporarily operated for a period of not more than 180 operating days in any calendar year at any site. Compliance with the opacity limits and various operating practices listed in the conditions of the AO shall be considered as application of Best Available Control Technology (BACT). The emission control measures listed in the conditions of this AO shall apply to all of the sites at which the equipment approved by this AO operates. Equipment in this AO is subject to the New Source Performance Standard (NSPS), 40 CFR Part 60, Subpart OOO. Hill Air Force Base (HAFB) has requested to install two (2) above ground fuel storage tanks, one (1) underground product recovery tank and one (1) fuel loading rack for the fuel hydrant system. This system will be located near the flight line and will improve the efficiency of fueling.
The estimated emissions increase will be 0.42 ton per year of Volatile Organic Compounds (VOC) and 0.02 tons per year of combined Hazardous Air Pollutants (HAPs). The total estimated emissions will be 0.42 tons per year of VOC and 0.02 tons per year of combined HAPs.
This operation is not subject to any current New Source Performance Standards (NSPS) and is not covered by the National Emissions Standard for hazardous Air Pollutants (NESHAP) for Aerospace Manufacturing and Rework Facilities (40 CFR 63 subpart GG). This Approval Order (AO) is being processed as an enhanced AO, and the Title V permit will be administratively amended after the AO is issued. HAFB is located in Davis County, which is a maintenance area for ozone, but is an attainment area for other criteria pollutants.

LeGrand Johnson Construction Company has requested to replace an old screen plant with a new larger screen plant at their Moab Aggregate Processing Plant. The old screening plant has a rating of 200 tons/hour and the new screening plant will have a rating of 250 tons/hour. The main pit location is approximately eight (8) miles south of Moab at 5101 East Old Airport Road. This plant is also approved to operate at all gravel pits currently being leased by the above stated company, within a three (3) mile radius of the main pit location. Current operations are being conducted approximately one (1) mile southeast of the main pit location on Spanish Valley Drive and not at the main pit. The emissions from the aggregate plant will be controlled by wet suppression methods.
The property is located in San Juan County, which is attainment for all criteria pollutants. New Source Performance Standard (NSPS) Subpart OOO (Standards of Performance for Nonmetallic Mineral Processing Plants) applies to this source. National Emission Standards for Hazardous Air Pollutants (NESHAP) and Maximum Achievable Control Technology (MACT) regulations do not apply to this source. This source is a Title V Area Source.
There will be no change in annual emissions, and the emissions will remain as the following, in tons per year, potential to emit totals: PM10 = 4.77, NOx = 28.94, SO2 = 3.75, CO = 9.69, VOC = 1.44.

AIR QUALITY
On March 6, 2009, Uranium One Exploration, Inc. submitted a Notice of Intent to replace a diesel engine-generator with a smaller unit. The currently permitted engine is capable of producing 1,600 kW which is greater than anticipated power demands. The replacement engine, which was previously located at the site, is much smaller, capable of producing only 500 kW.
Because the two engines are different models with different years of manufacture, there exists the possibility that emissions of some pollutants will increase. Uranium One Exploration, Inc. desires to operate the engine such that the overall Potential to Emit for all pollutants will not change.
The completed engineering evaluation and air quality impact analysis showed that the proposed project meets the requirements of federal air quality regulations and the State air quality rules. Approval is recommended.

WATER QUALITY
Coalville City Corporation Wastewater Treatment Plant is requesting a reissue of a UPDES Permit.
The facility serves the City of Coalville with a current population of about 1,470. The plant is designed for an average daily flow of 0.35 MGD. In addition to the discharge provisions, the renewal permit for Coalville will include provisions for biosolids.

AIR QUALITY
Granite Construction Company has requested a modification to their Approval Order (AO), DAQE-AN0104070015-08. It proposes to replace a baghouse associated with the BMG 45 R 30, drum mix asphalt plant, to add a Recycled Asphalt Pavement (RAP) facility, and to replace a 500 kW generator with a 725 kW generator. The site is located in Salt Lake County.
Salt Lake County is a Nonattainment area of the National Ambient Air Quality Standards (NAAQS) for PM10 and SO2, and is a maintenance area for Ozone. New Source Performance Standards (NSPS) 40 CFR part 60 - subparts A (General Provisions), I (Standards of Performance for Hot Mix Asphalt Plants), and OOO (Standards of Performance for Non-metallic Mineral Processing Plants) apply. National Emission Standards for Hazardous Air Pollutants (NESHAP) and Maximum Achievable Control Technology (MACT) regulations do not apply to this source. Title V of the 1990 Clean Air Act applies to this source.
The potential to emit totals, in tons per year (tpy), will change as follows: PM10 = +3.96, NOx = -9.07, CO = +9.11, VOC = +1.18
These changes in emissions will result in the following, in tpy, potential to emit totals: PM10 = 21.45, NOx = 49.61, SO2 = 4.10, CO = 46.21, VOC = 21.88, HAPs = 2.36.
The completed engineering evaluation and air quality impact analysis showed that the proposed project meets the requirements of federal air quality regulations and the State air quality rules. Approval is recommended.

Company Name: MABCO Memorials Incorporated
Location: MABCO Memorials Incorporated: Concrete Batch Plant – 489 West 12th Street, Ogden, UT, Weber County
Project Description: Bott Monument, owner and operator of the Ogden concrete product manufacturing plant has requested to modify their Approval Order (AO) to include additional equipment items. This modification is in response to a compliance inspection where the list of additional equipment was observed to be installed and in use but not listed in any AO. In the 1996 AO various unpaved haul roads are identified and several conditions listed to control fugitive dust emissions. Plant operations have been modified to pave plant property with concrete and minimize vehicle miles travelled on plant property. Therefore these conditions related to the unpaved haul roads are no longer necessary. Bott Monument also requests that the pressure drop observation condition associated with the baghouse that controls the emissions associated with the periodic filling of the cement storage silo, be removed. This silo is filled less than five times per year.
The emissions, in tons per year, will not change.
The following potential to emit totals, in tons per year are as follows:
PM10 = 0.25, NOx = 0.04, CO = 0.02, VOC = 1.17, HAPs = 0.44

R & K Marble, Inc., owner and operator of the Brigham City cultured marble manufacturing plant has requested to modify the existing AO by increasing production. As the demand for R&K Marble's product has increased the company anticipates consuming more of the raw materials that are identified with limitations in the general conditions of the AO. Specifically, the allowable consumption of styrene resin will increase from 3,500 gallons to 6,000 gallons per rolling 12-month period.
The emissions, in tons per year, will change as follows:
VOC (+ 0.55), HAPs (+ 0.55)
The changes in emissions will result in the following potential to emit totals, in tons per year:
PM10 = 0.02, VOC = 1.31, HAPs = 1.31

The Deseret Chemical Depot (DCD) will hold a public information meeting on July 22, 2009, at 6:00 p.m. at the Tooele Chemical Stockpile Outreach Office for a Class 2 permit modification request (09.02125). DCD proposes to add agent monitoring and quality control requirements for DCD storage area and perimeter monitors. These requirements are currently contained in the CAMDS permit, and DCD is requesting to transfer them to the DCD RCRA permit.
Comment period: 6/30/09 – 8/31/09.

AIR QUALITY
Questar Gas Company has requested a modification to their Approval Order DAQE-AN0432006-05 to remove the backup boiler testing requirements and lower hours of operations at the Salt Lake City Operation Center. The Salt Lake City Operation Center is located at 200 South 1140 West, Salt Lake City, Utah. The Salt Lake City Operation Center consists of three (3) existing internal combustion engines, one (1) existing boiler and one (1) existing emergency generator. There will be emission decreases from the Salt Lake City Operations Center as a result of this modification. The emission decreases from this modification are as follows: -0.01 Tons Per Year (TPY) of PM10, -2.4 TPY of NOx, -2.59 of TPY CO and -0.07 TPY of VOC. The emissions from the Salt Lake City Operations Center are as follows: 0.17 TPY of PM10, 17.37 TPY of NOx, 34.07 of TPY CO and 4.27 TPY of VOC.
The boiler is subject to 40 CFR 60 Dc. This is a minor source so Title V does not apply to this source. There is no emission increase associated with this modification so no modeling is required.
The completed engineering evaluation and air quality impact analysis showed that the proposed project meets the requirements of federal air quality regulations and the State air quality rules. Approval is recommended.
