Signs of Potential Environmental Problems
AIR

- Pollution control equipment that is not maintained (ex. – water sprays not working, overloaded or missing filters on spray booths, broken gauges, etc.)
- Decreased visibility as the result of activity at or near a site
- Dust blowing from a construction site
- Open burning
- Dumpsters or landfill areas with asbestos
- Asbestos materials visible at a demolition
- Peeling or chipped paint in homes and other structures built before 1978 and where children are present
- Portable equipment that has been relocated (crushers, concrete batch plants, asphalt plants) without a permit.
LAND
Hazardous Waste

- Open containers of chemicals or other hazardous waste
- Unlabeled/undated hazardous waste drums
- Unstable or leaking containers
- On-site hazardous waste storage beyond appropriate time limits
- Improper storage or disposal of hazardous waste
- Stained soils or areas of potential spills
Superfund
Contact: Division of Environmental Response & Remediation, (801) 536-4100
- At abandoned buildings or properties:
- Stained soil from spills
- Abandoned drums, transformers, or batteries
- Unusual, or a lack of, vegetative growth where it appears contamination may be present
- Exposed mill tailings or mine waste rock piles that may impact public health or the environment
- Oily sheen on puddled water
- Unusual odors
- Historical spills of hazardous chemicals or materials in soil or surface water.
Underground Storage Tanks

- Leaks at fuel dispensers and piping connections
- Spills from fueling tanks
- Unbarricaded tank excavations
- Alarms indicating possible UST system leaks
- Gasoline or diesel vapors in buildings and other structures
- Uncontrolled soil stockpiles (no berms or underlying plastic)
- Ground water monitoring wells with no covers
- Odd noises or discharges from remediation system stacks or pipes
- Petroleum contaminated soils discovered during construction or excavation near UST Vapors in utilities or utility trenches
Radiation Control

- Site or incident boundaries not maintained - (i.e.- roped off, visual surveillance, signs)
- Improper disposal of radiological waste
- Inappropriate labels or improper packaging of radiological materials
- Radiological materials that are not securely locked when not in use
- Unstored radioactive materials that are not under constant surveillance
- Radiation detection instruments are not used
- Personnel are not using protective devices (lead aprons, gloves, and shields)
- Equipment being used for purposes other than as intended
- Shipping papers and emergency procedures are not available during transportation
WATER
Drinking Water

In General:
- Unpleasant odor, taste, or aesthetic quality of water
- Chlorine odor
- Suspicious individuals or vehicles around water storage tanks or well houses
At a Spring:
- Surface water is running over the spring box/collection area
- Spring is not properly fenced.
- Animal grazing around the spring collection area
At a Well House:
- A hole or damage to the well casing or sanitary seal
- Well opening covered only by plywood
- Well casing is not 18 inches above ground
- The required pressure gauge, sample tap, and flow meter are not present
At a Water Tank or Storage Reservoir:
- Structural damage to a water tank (cracks in walls, leaks)
- Damage to the tubes that feed a line
- No screens on the storage overflow (to prevent from animals, insects, etc. from entering tank)
At a Distribution System:
- Asphalt sloughing off
- Bubbling water
- Ponding of water
Water Quality

- Wastewater discharge to surface waters or to storm sewers
- Industrial process wastewater discharge to a sanitary sewer
- Discharge waters are discolored or murky
- Process wastewater discharge to septic tank, pits, sumps, wells, leach fields, etc.
- Raw materials or products stored outside
- Uncontrolled storm water
- Construction site over one acre
- Construction site de-watering operation
- A facility where animals are stabled, confined, fed, or maintained and waste is not being controlled.
